If you decide to become a patient of our office you have two ways of completing the registration process. Call (615) 321-4740 or e-mail and request our new patient packet, or fill out the information on our secure web page. If you are completing the information on the web page you will still need to mail us your medical records from your previous physicians, and a copy of the front and back of your insurance card. Your first office visit will include a consultation, a review of previous medical records, history and physical exam, lab tests, and other procedures, as needed.
